Prepared, accurate documentation
Arrange medical forms and assessments in Toronto and learn what information, examination or testing may be required.
Instructions and every page help us identify the required examination and records.
Form visits may need more time than a routine appointment and are not walk-in paperwork.
The clinician can document only findings supported by the assessment and available information.
Many third-party forms and notes are not insured services; confirm costs before the visit.
Prepared, accurate documentation
Every organization asks a different medical question. Reviewing the complete form in advance helps determine whether the clinic can provide the assessment, how much appointment time is needed and what supporting information to bring.
A form is more than a signature. Depending on its purpose, completion may require a health history, focused examination, vision or hearing information, laboratory testing, records from another clinician or a separate appointment. A requested clearance, restriction or certification cannot be guaranteed.
Common requests
Availability depends on the form, the clinician’s scope and the records or tests required. Contact the clinic before assuming a particular document can be completed.
Assessment of current abilities, restrictions or readiness when requested by an employer or insurer.
Health information or participation forms based on documented history and current findings.
Medical information requested for benefits, claims or coverage, subject to consent and record review.
Health assessment for an Ontario driver form when appropriate and within the clinician’s scope.

Review for selected recreation, sport or program requirements when the necessary assessment is available.
Documentation based on a timely clinical assessment and the information reasonably available.
Organization-specific requests reviewed in advance for eligibility, complexity and clinician requirements.
The completion process
Starting early leaves time for records, tests or follow-up if the document cannot be completed safely in one visit.
Share the current form, issuing organization, purpose, instructions and submission deadline.
The clinic reviews availability, appointment length, records, tests and any applicable fee.
Discuss relevant history and complete the examination or measurements the document requires.
The clinician records supported findings; additional information may be needed before release.
Prepare for your appointment
A complete package reduces avoidable delays, but it does not guarantee same-day completion or a particular conclusion.
Bring every page, the current version and any standards supplied by the requesting organization.
Bring requested photo identification, health card and clear recipient or submission details.
Include prescriptions, allergies, relevant conditions, devices and recent specialist information.
Bring supporting reports or explain where previous tests and assessments were completed.
Understand the boundaries
The clinic documents medical information within the clinician’s scope. The employer, school, insurer, regulator or licensing body decides how it will use that information.
Work forms should focus on supported functional information relevant to safe duties and accommodations.
Read about the WSIB Functional Abilities Form →Medical reports must remain accurate, relevant and within the clinician’s expertise, even when a specific conclusion is requested.
Review CPSO third-party report guidance →Ontario’s Ministry of Transportation reviews medical information and determines licence status; the examining clinician does not make that administrative decision.
Ontario driver medical review information →Some forms, notes and third-party services are not covered by OHIP. Ask about the clinic’s fee and cancellation policy before your appointment.
Forms appointments are planned care
If you are acutely unwell, seek the level of care your symptoms require instead of waiting for a form appointment or administrative deadline.

Early review helps match the right appointment to the document and avoids promising an outcome that depends on clinical findings.
View all clinic FAQsSometimes, but not always. The clinician may need previous records, testing, a longer assessment or follow-up information before completing and signing the document.
Many forms, certificates and sick notes are uninsured services and may have a fee. Coverage can depend on the service and who requested it. Confirm the charge before booking.
No. The clinician can record only medically supported findings and opinions. The organization receiving the form makes its own eligibility, benefit, employment or licensing decision.
Contact the clinic first for approved delivery instructions. Do not send personal health information through an unconfirmed channel. Bring the complete original or a clear copy to the appointment.
The clinic will explain if the document is outside the available clinician’s scope or requires a designated provider. You may need to arrange the assessment with the appropriate external service.
