Start with the document—not just the deadline.

Every organization asks a different medical question. Reviewing the complete form in advance helps determine whether the clinic can provide the assessment, how much appointment time is needed and what supporting information to bring.

A form is more than a signature. Depending on its purpose, completion may require a health history, focused examination, vision or hearing information, laboratory testing, records from another clinician or a separate appointment. A requested clearance, restriction or certification cannot be guaranteed.

    The clinic reviews availability, appointment length, records, tests and any applicable fee.

    The clinician records supported findings; additional information may be needed before release.

A complete package reduces avoidable delays, but it does not guarantee same-day completion or a particular conclusion.

Bring every page, the current version and any standards supplied by the requesting organization.

Bring requested photo identification, health card and clear recipient or submission details.

Understand the boundaries

The assessment, the form and the final decision are different things.

The clinic documents medical information within the clinician’s scope. The employer, school, insurer, regulator or licensing body decides how it will use that information.

Some forms, notes and third-party services are not covered by OHIP. Ask about the clinic’s fee and cancellation policy before your appointment.

Forms appointments are planned care

Do not delay urgent care for paperwork.

If you are acutely unwell, seek the level of care your symptoms require instead of waiting for a form appointment or administrative deadline.
