Care that stays connected

Your first appointment
A first visit is usually a starting point. The clinician may need more than one appointment to understand a complex history or address several concerns safely.
Bring your Ontario health card and photo identification when requested.
Include prescriptions, non-prescription products, supplements and known reactions.
Bring reports you have available and the contact information for previous care providers.
Write down the concerns and health goals you would most like to discuss first.
Care while you are waiting
Choose the care option that matches the urgency and complexity of your concern. A registration inquiry is not a substitute for medical assessment.
Non-emergency concern
For an eligible recent illness, minor injury or focused concern, ask whether the walk-in clinic is the right option.

Availability and clinic policies can change. Our team will confirm the details that apply to your registration request.
View all clinic FAQsNo. The form begins an inquiry only. You are not registered or accepted until the clinic completes its process and confirms this with you directly.
Response times depend on inquiry volume and physician capacity. Submitting more than once does not improve your position and may slow review.
Each person generally needs their own inquiry because contact details, health-card information and registration decisions are individual. Ask the clinic about children or dependants.
If you are accepted, the clinic can explain its records-transfer process. Your authorization may be required, and your previous provider may charge an administrative fee.
Do not wait for registration. Ask about walk-in care for eligible non-emergency concerns, call 811 when you are unsure where to seek care, and use emergency services for severe symptoms.
